The Boot Sequence Is Set Once and Almost Never Revisited
Installing a server over the network is a normal way to build it, and it requires network boot to come first. After installation that order is usually left alone, because it is harmless: the local disk is found on the second attempt and the machine starts. The cost is a few seconds per boot, and a production server is rarely cold-started, so nobody ever pays it.

The provisioning service is eventually decommissioned, and at some point the power fails. Neither event is connected to the servers in anyone mind. But a machine looking for a service that no longer answers waits for a timeout, and where that timeout is long or the attempt repeats, the machine effectively does not start.
