Internal boot media is usually flash, and flash is consumed by writing. A hypervisor writes logs and state to its boot device continuously for years, and eventually the device reaches the end of what it can accept. It does not fail in the sense of breaking; it wears out. Nothing reports a fault because nothing is faulty, and then it is unwritable and the host will not start.
The workload does not, and the hypervisor does. Logging, state and configuration go to that device steadily regardless of how busy the virtual machines are. On a host that has been up for five or six years that is a great deal of accumulated writing onto a small, inexpensive device that was never sized for a decade.
Usually not, and that is the one piece of good news. Guest data lives on the array, not on the boot media, so the machines are intact and unreachable until the host can start. The recovery is replacing the boot device and restoring the host configuration, which is fast if you have that configuration and slow if you do not.
Treat internal boot media as a consumable with a service life rather than as a component that runs until it fails. We hold these modules for PowerEdge generations no longer sold, and replacing one during a planned window costs a fraction of discovering it during an unplanned one.

A hypervisor host that will not boot, with no error history and no component that has obviously failed, is often a host whose internal boot media has simply been used up. Flash storage is consumed by writing to it, and a hypervisor has been writing to that device quietly, every day, for as long as the machine has been in service.

Wear Is Not a Fault, So It Is Not Reported as One

Every flash device has a finite quantity of writing it can accept. A hypervisor host writes logs, state and configuration continuously, independently of how busy its guests are. Over five or six years of uptime that accumulates into a substantial total on a small, inexpensive module. The device does not break. It reaches its limit, stops accepting writes, and the host stops being able to start.

The workload gives no warning of it

Administrators reasonably judge write load by what the virtual machines are doing, and guests write to the array rather than to the boot device. So a host running light workloads accumulates the same hypervisor-level writing as a busy one. The device most likely to be consumed is on the machine nobody considers heavily used.

The guests survive, the availability does not

Guest data is on shared storage and is unaffected, which limits the damage considerably. What is lost is the host, and with it everything running on it until a replacement boot device is fitted and the host configuration restored. Where that configuration has been captured the recovery is short. Where it has not, an estate past its support date is rebuilding a host from memory while its services are down.
