Check what the chassis power configuration was set to. Redundancy is not automatic simply because two supplies are installed. Often nothing does, because nothing is faulty. Both supplies are healthy and the chassis is operating exactly as configured. Whether that configuration provides protection is a question about the setting and the current draw, and it is not one the hardware raises on its own.

Two Supplies Installed Is Not the Same as Redundant

A chassis holds a setting describing how its supplies are used: sharing the load, or one carrying while the other stands by, and what it should do as draw rises. Redundancy only exists if a single unit can carry everything the machine is drawing. Two supplies that are both contributing to a load neither could carry alone is a valid configuration and provides no protection at all.

Nothing is faulty, so nothing is reported

Both units are healthy, the chassis is doing exactly what it is configured to do, and the draw is within the combined capability. There is no error state anywhere in that description. Whether the arrangement survives a failure is a calculation, not a condition, and hardware does not generally alarm on the result of a calculation nobody asked it to make.

The upgrade that removed protection was not a power change

Machines grow. A card is added, disks are filled, processors are upgraded. Each increment raises the draw slightly and none of them is thought of as a power decision. At some point the total passes what one supply can carry, and the machine stops being redundant on a day when nobody touched the power system. There is no event to point at afterwards.
