An engineer reading only the switch they administer can therefore find a completely clean interface and conclude the problem lies elsewhere, which is how the wrong hardware ends up being replaced.

Hard-coding was usually a fix for something

Somebody hard-coded that port deliberately, often to solve an earlier negotiation problem with equipment that has since been replaced. The setting survives the equipment, and the replacement device negotiates perfectly well — which recreates the mismatch in reverse. Reading both ends after any device change is what catches it.
